District 109 offers a variety of benefits to staff, including medical and dental coverage and other wellness benefits, life and disability insurance, investment programs and other financial benefits. Staff can find complete benefits and payroll information on the D109 Intranet. 

Under the Illinois Freedom of Information Act (FOIA), access to District 109's public records is available to any person upon written request. Written requests for public records may be made via the form below, via e-mail, or may be submitted by personal delivery, mail or fax directed to the District 109 Freedom of Information Officer. Individuals making a request are not required to state a reason for the request other than to identify when the request is for commercial purpose or when requesting a fee waiver.

District 109 does not charge fees for Freedom of Information Act requests.

The FOIA Officer or designee shall approve all requests for public records unless:

1. The requested material does not exist;
2. The requested material is exempt from inspection and copying by the Freedom of Information Act; or
3. Complying with the request would be unduly burdensome.

The FOIA Officer will respond within five business days after receipt of a request (either comply with or deny the request), unless the time for response is extended as specified in Section 3 of FOIA.

Fraud involves intentional deception or misrepresentation to gain an unfair or unlawful benefit, often at the expense of an organization or individuals. This can include activities such as theft, financial misreporting, misuse of assets, falsifying documents, or other unethical behaviors that violate district policies or laws.

If you suspect any form of fraud within our District, please submit a detailed report to us.

Please include as much information as possible—specific dates, locations, individuals involved, and any supporting documentation or observations you may have. All reports are treated with confidentiality to ensure a secure reporting process.

Parents of District 109 students can visit the District Center to get work permits for their children. Work permits are issued to minors through age 15 years; a written application must be completed in order to obtain a work permit for a minor and is required under section 205./12 of the Illinois Child Labor Law. Requirements under the Illinois Child Labor Law include:

• Parent or guardian must be present at the time the application is filed
• The minor child to whom the permit will be issued must be present to obtain the work permit.

The following information must be provided for verification:

• The child’s social security card.
• A statement of intention to employ signed by the prospective employer listing the nature of the occupation in which he intends to employ the minor. The exact hours of the day and number of hours per day and days per week must be listed.
• Verification of age:
  • Birth Certificate furnished by the State or County or a signed statement of the recorded date and place of birth issued by a registrar of vital records or passport; or
  • A certificate of baptism duly certified showing the date of birth and place of baptism of the child.
• Phone number and current address.
• The date of the child’s last physical, which must be within one year of the date the minor is applying.
• During the school year only: The pink principal form verifying that the minor is making satisfactory academic progress to work part-time. This can be issued on the school letterhead and signed by the principal.
• The work permit form (issued by the Illinois Department of Labor), available from the District Center.
